Registered Nurses

When you or a loved one needs support from a medical professional due to aging, dementia, post-surgery or another circumstance, you want the very best care available. Luckily, there are a variety of in-home Registered Nurse options available to families. Home nursing care for elderly patients is often times the ideal situation for loved ones. In-home nursing care with a Registered Nurse (RN) is an option that’s not only available, but also one that might be more affordable than you think. Furthermore, Registered Nurses can provide a higher level of care when needed. Unlike many other in-home medical professionals, Registered Nurses are licensed to provide a higher level of care to patients.

Licensed Vocational Nurse

We have licensed vocational nurses who can provide part time or full time shift care, as well as intermittent visits, depending on the patient's needs. Our nurses do a complete initial evaluation and also function as case managers to ensure that our patients get all the services they need and that those services are coordinated appropriately. Our goal is to improve the patient's health condition while helping the patient to stay in the security and comfort of their own home. All of our services are provided pursuant to a doctor's order.

Certified Home Health Aides

All of our home health aides are certified by the State of California. Home health aides help the patient with the activities of daily living, such as bathing, grooming dressing, and eating. They assist the patient with transferring from bed to chair to standing and walking and back again. They may also help with food preparation and keeping the home environment safe.

Physical Therapy

Our physical therapists find it very rewarding as they assist people to reclaim their mobility, independence and daily routines in their own home. The advantages of receiving physical therapy at home are mastering the daily tasks which take place in the home, and to increase safety. It’s in the home setting where most accidents occur and having the PT there allows them to assess home safety, set functional goals, and work with the patient to maximize their ability, improve strength, balance, coordination, and make strides by enhancing their quality of life as their physical impairment declines and mobility increases.

Occupational Therapy

Occupational therapy (OT) is a patient-centered practice with a focus on strategies that develop, modify, return or help to maintain essential activities of daily living. Spirit Home Health’s OT recognizes that the best place to get better is at home. When performed in the home, individuals have the advantage of learning skills that are pertinent and useful in their everyday lives.

Speech Therapy

Spirit Home Health’s speech/language pathologists provide in-home assessment and interventions for conditions such as Parkinson’s disease, stroke, ALS, head injury, dementia, COPD and CHF or anything that results in an individual losing the ability to form words and understand language or developing speech disorders or a difficulty in swallowing. This can seriously hinder that individual’s ability to continue living independently in a safe and healthy manner. The patient, caregiver and their family are encouraged to ask questions, make comments and receive appropriate trainings from our speech therapists.

Medical Social Worker

Spirit Home Health’s Medical Social Workers evaluate and assist clients/patients and families to help them adjust to the social and emotional factors related to their health challenges in their place of residence. MSWs make assessments and work to understand each patient’s personal goals and most pressing needs. They help patients and their families cope with issues by connecting them with community resources, educating on options, and facilitating discussions between the patient and their family.
