Home Health Nursing

A licensed nurse provides part time or intermittent care to the homebound patient who is determined by the Physician to have acute or chronic medically predictable, recurring needs which require treatment such as observation and assessment, care of wounds, administration of intravenous solutions and patient teaching.

The types of chronic disabling conditions treated in the home include:

• Circulatory problems • Neoplasm
• Heart conditions • Diabetes
• Stroke • Musculoskeletal problems
• Chronic obstructive pulmonary disease

An acute condition may be an exacerbation of a chronic problem, an early release from the hospital or patient request for home therapy versus hospitalization. An important alternative to hospital care, home care has become a critical component in the continuum of care for cancer patients and many post operative situations.

Basic home health care offers the patient personal care services which help provide and maintain activities of daily living, emotional comfort and assist the patient toward independent function. Assistance is provided in personal hygiene and self-care activities such as grooming, bathing, dressing and feeding. When medically necessary, patients are assisted with laundry, grocery shopping, food preparation, general cleaning and environmental safety.
